Disclaimer Regarding Records Maintained by Third Parties
SIMEDHealth’s obligation to provide access to medical records applies to the records created and maintained by SIMEDHealth or stored within SIMEDHealth’s designated electronic health record system. Certain services provided by third-party or contracted providers, including physical therapy and other ancillary services, may maintain their clinical documentation in external systems not owned or controlled by SIMEDHealth. In those instances, SIMEDHealth will assist the patient in accessing those records by providing the appropriate contact information or facilitating the request as permitted by law, but SIMEDHealth is not the custodian of those external records.
Separation of Record Systems
Some clinical services performed within SIMEDHealth facilities are documented using separate record systems maintained by the service provider. While these records relate to the coordination of patient care, they may not constitute part of SIMEDHealth’s designated medical record set for HIPAA access purposes unless incorporated into the SIMEDHealth record.
